2012 Spring Registration

Late fee applies to registration on this date ($15.00)

Registration Fees
Single Participant Any Additional Siblings	$95.00 $55.00 each
Field Cleanup Fee	$20.00
This fee is required for each family participating at RRLL. This fee will be reimbursed if you participate and attend one of the scheduled clean up days at RRLL. This amount can be paid in both cash deposit or included in your general registration fee. If you would like the $20.00 fee held and returned, a separate check will be required.
Late Registration Fee	$15.00
This fee will be added to any registrations after the last scheduled registration date of 1/28/2012.
Returned Check Fee:	$25.00
Any returned check will require a $25.00 returned check fee in addition to original registration fee. Player will not be allowed to try out until fee is paid and verified.

Please fill out the following forms and bring them to registration along with 3 proofs of residence and ORIGINAL birth certificate.

REGISTRATION FORM MEDICAL RELEASE FORM VOLUNTEER APPLICATION

In accordance with National Little League rules, players must provide (3) proofs of residence and an original birth certificate*. This is true even for returning players. THERE ARE NO EXCEPTIONS!

Valid proofs of residence include:

current lease agreement
current mortgage documents
current cable or electric bill

We will not accept water bills, your phone bill (unless your name and address is visible), checks.

We will not accept baptismal certificates in lieu of birth certificates.

We will not accept photocopies of a birth certificate.

If you do not have the correct items when you come in to register, your application will not be processed and you will be sent away until you have the proper documents. Please fill out the Registration and Medical Release forms for each player (these forms can be completed online, printed and hand delivered). If you would like to coach, manage or be an umpire you will also need to fill out a Volunteer Application.

Remember every team needs umpires and scorekeepers so please volunteer./font>

* Since we do not keep copies of birth certificates, we must check the original birth certificates each year for all players.
